Would be the first I've seen or heard of it,
Unless it was knocked off I don't think it could.

The TAF at NLP PlayExpo last year had a book of some sort that had fallen off the bookcase and was jamming up the vault entrance. Tonnes of people were playing it oblivious, surprised the bookcase motor didn't burn out. I still managed to get a better score on that machine than I've ever got on my own TAF despite the vault being inaccessible. Reported it to be fixed. Not a worry in the home but something to watch out for if you take it to a show.
