This is a Fliptonics I board from The Addams Family, fully tested and working.
One of the four TIP36C transistors went bad and I have replaced it with a new one from Andy Netherwood (top right). It looks like one of the TIP102s (Q5) has also been replaced at some point in the past by a previous owner.
